Basophils are the rarest of the five main types of white blood cells, normally making up 0% to about 1% of the white cells circulating in your blood. Despite their scarcity, they punch above their weight: their granules store histamine, heparin, and other signaling chemicals that drive allergic reactions and help coordinate inflammation. When you have hives, hay fever, or certain parasitic infections, basophils are among the cells sounding the alarm.

You'll almost never see a basophil test ordered on its own. The number appears automatically as part of the white blood cell differential on a complete blood count (CBC) — usually labeled "Basos," "BASO," or "Basophils %." Because the count is so small, most results read 0% or 1%, and a zero is completely normal. That scarcity also means the percentage is a jumpy number: a change of a few dozen cells per microliter, or a shift in your other white cells, can move it noticeably.

What makes basophils worth understanding is the high end. A persistently elevated basophil percentage — especially above 2% — is called basophilia. Most of the time it traces back to everyday causes like allergies, an underactive thyroid, or chronic inflammation. Rarely, sustained basophilia alongside other abnormal blood counts can be an early clue to a bone marrow disorder such as chronic myeloid leukemia, which is why doctors pay attention when the number stays high across repeat tests rather than reacting to a single reading.

How to read your result

Reading basophils is asymmetric: the low end barely matters, and the high end matters mostly when it persists. Because the normal range starts at 0%, there is no meaningful "low basophils" result on a percentage test — a zero is expected in many healthy adults, and true basopenia (from steroids, hyperthyroidism, or acute infection) generally can't be diagnosed this way. On the high side, first check your own lab's range, since normal tops out at 1% at some labs and 2% at others. A value just over the line with an otherwise normal CBC is usually noise, allergy, or recovery from illness. A value above 2% earns a second look: confirm it with the absolute basophil count (elevated above roughly 0.2 ×10⁹/L), repeat the CBC in a few weeks, and pay attention to whether the total white count, platelets, or other lines are also abnormal — that combination, not the basophil number alone, is what guides any further workup.

When it's tested

Basophils are included automatically in any complete blood count (CBC) with differential — the standard blood panel ordered at annual physicals, before surgeries, and during workups for infection, fatigue, allergy, or unexplained symptoms. They are not part of a basic metabolic panel (BMP/CMP) or a lipid panel. If a differential result looks unusual, clinicians may follow up with a repeat CBC, an absolute basophil count, or a manual blood smear review.

The basophil percentage is a proportion, so it has no SI conversion; the related absolute basophil count appears as cells/µL in US reports or ×10⁹/L in SI reports (1,000 cells/µL equals 1.0 ×10⁹/L).
