What 90 % means

A lymphocyte share of 90% or more in an adult means the other white cell types — especially neutrophils, normally the most numerous — have almost disappeared from the differential. That happens in two very different ways. Either neutrophils have genuinely collapsed (agranulocytosis, most often from a drug reaction, chemotherapy, or a bone marrow problem), leaving even a normal number of lymphocytes as essentially the only cells being counted — a situation that leaves you highly vulnerable to serious bacterial infection. Or lymphocytes (or immature lymphoblasts, which some analyzers group with them) have multiplied enormously, as in advanced chronic lymphocytic leukemia or acute lymphoblastic leukemia. The percentage itself is not a defined lab panic value — those are set on absolute counts — but the patterns that produce a number like this are serious, and the total WBC, absolute neutrophil count, and absolute lymphocyte count on the same report are what distinguish them.
Most US labs consider roughly 20% to 40% of white blood cells a normal lymphocyte share for adults (the convention used by MedlinePlus and NHLBI), though some labs extend the range to about 18-45%.
Common causes
- Agranulocytosis or profound neutropenia — from drug reactions (for example methimazole, clozapine, some antibiotics), chemotherapy, or marrow disease — leaving lymphocytes as nearly the only cells counted
- Chronic lymphocytic leukemia (CLL) with a very high lymphocyte count
- Acute lymphoblastic leukemia, where lymphoblasts may be counted with lymphocytes on an automated differential
- Other lymphoproliferative disorders, such as prolymphocytic leukemia
